Services include maternal/child health; primary care; Elder Screen; WIC; Twelve-Teen Program; First Steps & Teen Pregnancy Coordinator; Community Health Nursing; and Community Health Representatives. This department functions as a health clinic, focusing on medical diagnosis and appropriate treatment and/or referral.

Services are for those who are at risk for a preventable visit to the hospital emergency department or for calling 9-1-1; either because they do not know how to access the appropriate help or resources or have not accessed or refused the appropriate help. Conexiones offers culturally- and linguistically-specific psychological evaluations and counseling for Latino children and adults. Evaluations are offered for cognitive functioning, attention problems (ADHD), disabilities, autism, and learning problems or disabilities for individuals from kindergarten age to college/university age.

For children and adolescents who are exhibiting problems of mood, behavior or adjustment. Child Mental Health Specialists work with children individually, in groups, and in the context of their families to improve behaviors, promote positive change and strengthen families' connections with ongoing community supports. Peer supports for parents and a parent Wellness Group are available.
