Eligibility

Children and youth in Grant County who:
  • Are 20 years old or under
  • Have complex mental or behavioral health needs
  • Are Medicaid-eligible
  • Complete a CANS screening (Child and Adolescent Needs and Strengths tool)
  • Meet Access to Care Standards, which determine the right level of care

Hours

M-F, 8am-noon, 1-5pm.

Application process

Call or complete online form to request a screening. Youth ages 13 and older can ask for a WISe screening directly. A parent or legal guardian must give permission for youth ages 12 and younger. A parent or legal guardian can also request a screening for a youth ages 13 and older, but they must agree to participate.

Offers intensive mental health services to support youth and their families who are experiencing complex behavioral health challenges. Services are delivered by a collaborative team and are designed to help families thrive in the least restrictive setting possible, including at home, at school, or in the community. Brings together natural supports (like family members, friends, or faith leaders) and professional team members (therapists, care coordinators, school staff, probation officers, etc.) to build a plan tailored to the child and family’s needs.
