Children in Grant County who:
  • Are 20 years old or under
  • Have complex mental or behavioral health needs
  • Are Medicaid-eligible
  • Complete a CANS screening (Child and Adolescent Needs and Strengths tool)
  • Meet Access to Care Standards, which determine the right level of care

M-F, 8am-5pm.

Offers a voluntary, home- and community-based program that supports youth and their families who are experiencing complex behavioral health challenges. Services are delivered by a collaborative team and are designed to help families thrive in the least restrictive setting possible—often right in their home or community. Brings together natural supports (like family members, friends, or faith leaders) and professional team members (therapists, care coordinators, school staff, probation officers, etc.) to build a plan tailored to the child and family’s needs.
