Offers a voluntary, home- and community-based program that supports youth and their families who are experiencing complex behavioral health challenges. Services are delivered by a collaborative team and are designed to help families thrive in the least restrictive setting possible—often right in their home or community.
Brings together natural supports (like family members, friends, or faith leaders) and professional team members (therapists, care coordinators, school staff, probation officers, etc.) to build a plan tailored to the child and family’s needs.
