Lincoln County families with children under the age of 18 who may have or be at increased risk for chronic physical, developmental, behavioral, or emotional conditions. Their complex needs mean many areas of their life may be impacted.

Lincoln, WA
Provides care coordination for families with children under the age of 18 who have special healthcare needs. A public health nurse assesses the child and family's needs, helps find resources, programs, services, and other supports that can help the child and family. May provide financial assistance for medical services and equipment. Nurses meet with the families at the office or come to the family's home and also provide support over the phone.
