Provides integrated care coordination for the Apple Health eligible population who are in out-of-home placement through the Department of Children, Youth, and Families. Offers
  • Help finding local Medicaid providers
  • Explains Medicaid coverage the client qualifies for
  • Help with prescriptions and prior authorizations issues
  • Answers health and behavioral/mental health questions
