Provide guidance, information, and referrals to programs and services that meet the needs of older adults and people with disabilities. Assists in the following areas:
  • Help at Home: Find support for daily tasks like bathing, dressing, meal prep, transportation, and personal care. Staff can connect you with both private and state-funded providers.
  • Medicaid Support: Get information about Medicaid long-term care coverage and help with the application process.
  • Support for Caregivers: Programs that give family caregivers a break, advice, and guidance on their own needs.
  • Long-Term Care Options: Learn about assisted living, adult family homes, nursing homes, and respite care facilities.
  • Other Resources: Help with things like property tax deferrals, powers of attorney, living wills, advance directives, transportation, and parking permits for people with disabilities.
